5K/2M Run-Walk for Education – Participant Waiver and Release of Liability

By signing below (or registering online), I acknowledge that I have voluntarily chosen to participate in the Ossining MATTERS Education Foundation Run/Walk for Education (the “Event”). I understand and agree to the following terms:

Assumption of Risk
I understand that participation in the Event involves physical activity, which carries inherent risks including, but not limited to, falls, contact with other participants, weather conditions, traffic, and road conditions. I voluntarily assume all risks associated with participation in this Event.

Release of Liability
In consideration of being permitted to participate in the Event, I, on behalf of myself, my heirs, executors, administrators, and assigns, do hereby release and discharge Ossining MATTERS, Inc., The Town and Village of Ossining, The New York State Department of Parks and Recreation, The Avalon, The Ossining Recreation Department, Super Race Systems, The Center at Mariandale, all event sponsors, organizers, volunteers, officials, and their representatives from any and all claims or liabilities, including but not limited to personal injury, illness, property damage, or death arising out of or in connection with my participation in this Event.

Medical Treatment
I certify that I am physically fit and sufficiently trained to participate in the Event. I consent to receive medical treatment that may be deemed necessary in the event of injury, accident, or illness during the Event.

Photographic Release
I grant permission to Ossining MATTERS. Inc. and its representatives to use any photographs, videos, or recordings of me taken during the Event for promotional purposes in any media, without compensation or additional approval.

Minors
If registering a minor (under 18 years old), I certify that I am the parent or legal guardian and grant permission for the child to participate. I also agree to the terms and conditions of this waiver on their behalf.

Acknowledgment
I have read this waiver and release of liability carefully. I fully understand its contents and voluntarily agree to its terms by signing below or completing registration online.
