In consideration of you accepting this entry, I, the participant, intending to be legally bound do hereby waive and forever release any and all rights and claims for damages or injuries that I may have against the Event Director, RunSignup.com, and all of their agents assisting with the event, sponsors and their representatives, volunteers and employees for any and all injuries to me or my personal property. This release includes all injuries and/or damages suffered by me before, during or after the event. I recognize, intend and understand that this release is binding on my heirs, executors, administrators, or assignees.

I know that Pickleball is a potentially hazardous activity. I should not enter and participate unless I am medically able to do so and properly trained. I assume all risks associated with participating in this event including, but not limited to: falls, contact with other participants, the effects of weather, traffic, and court conditions, and waive any and all claims which I might have based on any of those and other risks typically found in a Pickleball Tournament. I acknowledge all such risks are known and understood by me. I agree to abide by all decisions of any tournament official relative to my ability to safely compete in the tournament. I certify as a material condition to my being permitted to enter this tournament that I am physically fit and sufficiently trained for the completion of this event and that a licensed Medical Doctor has verified my physical condition.

In the event of an illness, injury or medical emergency arising during the event I hereby authorize and give my consent to the Event Director to secure from any accredited hospital, clinic and/ or physician any treatment deemed necessary for my immediate care. I agree that I will be fully responsible for payment of any and all medical services and treatment rendered to me including but not limited to medical transport, medications, treatment and hospitalization.

As it applies to my participation in this race, I agree to abide by the Center for Disease Control (CDC)’s recommendations for the prevention of the spread of COVID-19 and attest to having read the CDC’s guidance at: https://www.cdc.gov. I also agree to abide by any COVID-19 distancing and other safety guidelines issued by the state, the community or by this race for my participation in this race.

Further, I grant permission to all the foregoing to use my name, voice and images of myself in any photographs, motion pictures, results, publications or any other print, videographic or electronic recording of this event for legitimate purposes.

This event follows the standard tournament industry policy: All entry fees are non-refundable. We reserve the right to postpone or cancel the event due to circumstances beyond our control such as a natural disaster or emergency or as required to protect the safety of participants and staff. No refunds will be issued under these circumstances. We reserve the right to change the details of the event without prior notice. I understand that my entry fee is nonrefundable and teams are non transferable.

By submitting this entry, I acknowledge (or a parent or adult guardian for all children under 18 years) having read and agreed to the above and below release and waiver including the no refund policy.

Accident Waiver and Release of Liability

I hereby assume all of the risks of participating in this GLOW Pickleball Tournament, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained or controlled by them, or because of their possible liability without fault. I certify that I am physically fit and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the organizers of this GLOW Pickleball Tournament in which I may participate and that it will govern my actions and responsibilities at this activity. In consideration of my application and permitting me to participate in this GLOW Pickleball Tournament, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: (A)I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from the City of Olive Branch facility for the GLOW Pickleball Tournament THE FOLLOWING ENTITIES OR PERSONS: The Event Benefits LLC, the City of Olive Branch, Light Garden, the Fellowship of Christian Athletes agents, representatives or volunteers. (B) I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this GLOW Pickleball Tournament, whether caused by negligence or otherwise. I acknowledge that this GLOW Pickleball Tournament may carry with it the potential for death, serious injury, and personal loss. The risks may include, but are not limited to, those caused by facilities, condition of participants, equipment, actions of other people including, but not limited to, participants, volunteers, spectators and lack of hydration. I consent and agree that event representatives or volunteers may take photographs or digital recordings of me as I participant during this event and use these in any and all media for promotional purposes. I further consent that my identity may be revealed therein or by description text or commentary. I waive any rights, claims or interest and I understand that there will be no financial or other remuneration. The accident waiver, release of liability and image release shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

CODE OF CONDUCT

** Always display behavior that demonstrates good sportsmanship, respect, fairness, integrity and honesty to other players and spectators.

** Refrain from using obscene language or excessive anger (i.e. throwing a paddle) during play.

** No participant or spectator shall display behavior or action that constitutes harassment, where harassment is defined as verbal, written, digital or physical behavior directed towards an individual, which is offensive, abusive, racist, sexist, degrading or malicious.

** Always strive to make correct line calls, but if in doubt, call the ball "in".
