As a participant in the Run/Walk, I recognize and acknowledge the potential risks of physical injury. I agree to assume full risk of any injury, damage or loss which I may sustain as a result of participation in any and all activities connected with such program(s). I agree to waive and relinquish all claims I may have against Taylorville Memorial Hospital (TMH) and its officers, agents, servants and employees as a result of participating in the program(s). I do hereby fully release and discharge TMH and its officers, agents, servants and employees from any and all claims from injuries, damages or loss which I may have or which may accrue to me on account of my participation in the program(s). I further agree to indemnify and hold harmless and defend TMH and its officers, agents, servants and employees from any and all claims resulting from injuries, damages and losses sustained by me arising out of, connected with or in any way associated with the activities of the program(s). I further grant full permission to TMH to use any photographs or videos of the event for any reasonable purpose. I have read and fully understand the above Waiver of Liability.
