In consideration of you accepting this registration, I, the parent or legal guardian of the participant (the "Camper"), intending to be legally bound, do hereby waive and forever release any and all rights and claims for damages or injuries that I or the Camper may have against the Camp Director, Memphis Youth Athletics, and all of their agents, staff, sponsors, representatives, volunteers, and employees for any and all injuries to the Camper or their personal property. This release includes all injuries and/or damages suffered before, during, or after camp participation. I recognize, intend, and understand that this release is binding on myself, the Camper, and our heirs, executors, administrators, or assignees.
I understand that participation in a youth summer camp involves a variety of physical activities, including but not limited to running, games, outdoor play, sports, crafts, field trips, and instructional activities. I acknowledge that these activities carry certain inherent risks, including but not limited to: trips and falls, weather-related injuries, interactions with other participants, insect bites, sun exposure, and transportation-related risks during off-site trips. I certify that the Camper is physically able to participate in all camp activities and that a licensed medical professional has cleared them for participation, if necessary.
In the event of an illness, injury, or medical emergency arising during camp, I hereby authorize and give my consent to the Camp Director and/or their designees to secure from any licensed hospital, clinic, or physician any treatment deemed necessary for the Camper’s immediate care. I understand that I will be fully responsible for the cost of any medical treatment or services rendered, including but not limited to medical transport, medications, emergency care, and hospitalization.
As it relates to participation in the camp, I agree to follow any current public health guidelines, including those provided by the Centers for Disease Control and Prevention (CDC), and any additional health and safety protocols implemented by the camp organizers related to COVID-19 or other communicable illnesses.
Furthermore, I grant permission to Memphis Youth Athletics and its representatives to use the Camper's name, voice, and image in any photographs, videos, publications, social media, or other promotional materials created during camp for legitimate purposes.
I understand that all camp registration fees are non-refundable. The camp reserves the right to postpone or cancel any camp days or sessions due to weather, emergencies, public health orders, or other circumstances beyond its control, and no refunds will be issued in such cases. Program details are subject to change without notice.
Memphis Youth Athletics Zero Tolerance Policy on Fighting, Bullying, and Abuse
At Memphis Youth Athletics, the safety and well-being of every child in our care is our highest priority. We are committed to maintaining a positive, respectful, and inclusive environment where all campers feel safe and supported.
As such, we enforce a zero tolerance policy for any form of fighting, bullying, physical aggression, or verbal abuse.
Policy Guidelines:
Fighting, whether physical or verbal, will not be tolerated under any circumstances.
Bullying, including teasing, exclusion, intimidation, or any repeated harmful behavior—whether in person or online—is strictly prohibited.
Physical or verbal abuse of any kind directed toward another camper, staff member, or volunteer is grounds for immediate disciplinary action.
Consequences:
If your child is involved in any such incident, you will be contacted immediately after the situation is addressed.
Your child will be sent home for the remainder of that day.
Depending on the severity of the incident, or if your child is a repeat offender, they may be removed from the camp for the remainder of the session or the full summer.
This policy exists to protect all participants and ensure a safe, enriching experience for everyone. By registering for camp, you acknowledge and agree to abide by this policy.
By submitting this registration, I acknowledge that I have read and agreed to the above release and waiver, including the no-refund policy. I certify that I am the legal parent or guardian of the participant and am authorized to sign this waiver on their behalf.
